You’ve been asking similar questions for over a year. You will not learn the intricacies of good software and systems development practices from a general forum like this. At a minimum, you need several University level classes or industry development classes.

If you want to learn about software development maturity, go to www.sei.cmu.edu and read about the Capabilities Maturity Models. Perhaps you should take a course or two in the basics of the CMMi. They offer several online from the site. They also list the names and addresses of certified assessors and trainers in your area who can assist you.

Once you take these classes, you will understand about Requirements Management, Requirements Development, Change Control Management, and Configuration Management. You will understand that developers ONLY develop to standards (we call them requirements) and mature organizations maintain an audit trail.

Developers follow these practices not because they are forced to, but because good practices make the job easier. You will also learn why, when organizations follow these practices, it is very difficult for “rogue developers” or destructive personality types to bring down a large scale project (a la Collins) singlehandedly.
